Myocardial Viability Imaging

چکیده

The concept of hibernating myocardium originated from observations in the 1970s that the myocardial dysfunction seen in patients with chronic coronary artery disease (CAD) was not always permanent (1). The fact that revascularization with coronary artery bypass grafting (CABG) could result in the recovery of contractile function in previously akinetic segments led to the development of numerous imaging techniques to differentiate viable from nonviable myocardium. Although enhanced regional and global left ventricular (LV) function can be demonstrated and often translates into symptomatic benefit in patients with severe ischemic LV dysfunction, the impact on survival of identifying and then revascularizing viable but dysfunctional myocardium remains unclear. Previous outcome studies using various imaging modalities to assess viability were small and retrospective in nature, and predated the era of modern medical therapy (2,3). Only recently have 2 prospective, randomized trials comparing medical therapy to surgical or percutaneous revascularization based on preintervention viability testing been completed and reported,

Serial Myocardial Imaging after a Single Dose of Thallium-201

Although thallium-201 exercise scintigraphy has been established for the detection of myocardial ischemia and viability, little is known regarding the myocardial thallium-201 kinetics during angioplasty. Herein, we report a 77-year old man with angina pectoris, in whom serial myocardial imaging after a single dose of thallium-201 was helpful in identifying not only the culprit lesion and myocar...

The value of myocardial perfusion imaging with Tc-99m MIBI for the prediction of perfusion improvement after percutaneous transluminal coronary angioplasty

Introduction: Percutaneous transluminal coronary angioplasty (PTCA) is an effective method for revascularizing of stenotic coronary vessels. Lack of response to this treatment, either in symptomatic or asymptomatic patients, is usually due to incomplete revascularization, restenosis, and/or irreversibility of myocardial perfusion. Introduction of a noninvasive metho...

Filter selection for 99mTc-Sestamibi myocardial perfusion SPECT imaging [Persian]

Myocardial perfusion SPECT imaging with Tc99m-sestamibi is the most accurate non-invasive means of detecting coronary artery disease and assessing the severity of perfusion abnormalities in the patients with coronary stenosis. Though simple and straight forward the results produced by the technique is very much affected by the details being used. One of the main problems with the test, wh...

Late gadolinium enhancement imaging in assessment of myocardial viability: techniques and clinical applications.

Assessment of myocardial viability is of ever-evolving interest in cardiovascular imaging, with major societies having incorporated viability imaging as class I or class IIa indications in their guidelines to better guide patient management. As with late gadolinium enhancement cardiac magnetic resonance (MR), assessment of residual myocardial viability or the extent of myocardial infarction is ...

Non-invasive imaging in detecting myocardial viability: Myocardial function versus perfusion

Coronary artery disease (CAD) is the most prevalent and single most common cause of morbidity and mortality [1] with the resulting left ventricular (LV) dysfunction an important complication. The distinction between viable and non-viable myocardium in patients with LV dysfunction is a clinically important issue among possible candidates for myocardial revascularization. Several available non-in...

CT imaging of myocardial viability: experimental and clinical evidence

Over the last decade, imaging of myocardial viability has become a well-established indication in patients suffering from myocardial infarction. Myocardial viability imaging is routinely performed using (18)F-fluorodeoxyglucose positron emission tomography, single-photon emission computed tomography or magnetic resonance imaging. Only recently have several multi-slice spiral computed tomography...
